Q: My doctor said I had more lines on my right forehead than my left so injected more areas on my right than my left. Now my upper eyelid on the right side has dropped. What can I do?A: You may have had more lines because the right upper eyelid is hanging lower and your forehead is actively raising the forehead to help pull it up. Then when the Botox™ relaxes the forhead, the eyelid comes down with the forehead. It is possible that a little more Botox™ in the glabella and outer eyebrow margin may help lift this side. Be certain to discuss your concerns with your physician as they may be able to repair this. If they can't, at least they will want to see how you responded so they know how to adjust the needle sites for the next treatment.
Q: I wanted to reduce the vertical lip creases and my doctor suggests Botox™ and fillers. Is this reasonable?
A: Without examining you I can only comment that Botox™ is used, off-label, to minimize pursing of the muscles in some individuals so that the lines become smoother. It also helps the filler last longer, we think because there's less muscle activity stimulating the degradation of the filler.
Q: My friend wants to thin out her lower face which she finds is too fat. In fact, there doesn't seem to be much fat on her anywhere. Her doctor said her muscle is too big and to use Botox™. What does this mean?
A: The masseter muscle is a major chewing muscle and brings the maxilla and mandible bones together so that the jaws can bite. Some people are born with a tendency to develop a hypertrophic, or enlarged, masseter mscle. The Botox™ can be injected into the muscle and shrink its girth. Certainly this is not permanent and treatment must be done every several months for maintenance. See an expert because there can be side effects.
Q: Can a cosmetic cream work as well for wrinkles around the eyes as Botox™?
A: I don't think there will be as good a treatment for the crows' feet as Botox™. The muscle of the eyelid area makes these lines as we squint and smile. Creams may help the lines look better, but unless you relax the muscle, the lines reform quickly.
