Q: I have lost ten pounds with exercise but can't get rid of my lower tummy, no matter what I do. Can Zeltiq's CoolSculpting in NYC help?

A: If you have an abundance of a fat depot overlying the stomach's muscular wall, then CoolSculpting could be for you. However, if your muscle wall has a weakened outer sheath of connective tissue, and the strong muscle wall is bulging forward without much overlying fat, then you would need an abdominoplasty (tummy tuck) to get a flatter stomach. You will need an examination to determine what is best for you.

Q: Can NYC CoolSculpting by Zeltiq be done over scar tissue?

A: In consultation, I examine scars to ensure that there is no obvious hernia and that the scar is mature and strong. We have had good results in patients who have had abdominal scars. There can be a greater chance, though, of scar sensitivity and increased bruising after the CoolSculpting.

Q: Are there different devices to do NYC CoolSculpting?

A: Not at this time. There is one company, Zeltiq, that has produced cryolipolysis. Freezing the fat by CoolSculpting produces dead fat cells that get digested by the body. There is about 20% reduction in fat that is seen in four months for the average person that was proven by ultrasound investigation during the studies when CoolSculpting got the FDA clearance.

Q: How safe is CoolSculpting by Zeltiq. I want to have my NYC dermatologist do it to reduce my fat stomach but I don't want liposuction because I have a clotting problem. Should I?

A: Zeltiq is a non-invasive method of freezing the fat and has been FDA-cleared. This very commonly done procedure may produce some soreness bruising and even numbness but these seem to be temporary symptoms. Have your dermatologist speak to your hematologist about your clotting problem as you might have a much greater bruise than average and there could be a theoretical risk in you of developing a hematoma (collection of blood in the fatty layer or dermis) that might need to be drained after the treatment. I am not aware of any complication having occurred such as this from CoolSculpting but it would have to be addressed prior to the treatment.

Q: I am 40 pounds overweight. Can I have CoolSculpting on my love handles? My NYC dermatologist said that the Zeltiq treatment can help.

A: Yes, you can, but the small improvement, even after a few treatments, may not be as dramatic as if you were in better shape.
