Q:I had bad acne when I was a teenager. Now my scars really bother me. I don't want to date in NYC because I feel really self-conscious. Is Fraxel helpful? Is there a really good treatment for acne scars that works?

A: There can be several treatments that work well together including punch grafts, scar revisional surgery, and Fraxel Restore or Repair resurfacing. Dermabrasion also can help immensely. Fraxel Restore, done as a series of multiple treatments, about three weeks apart, can leave some redness and minute swelling but doesn't create an absolute down time. There is improvement as new collagen is produced and the dermis is fortified making the scars look better from the bottom up, or inside-out.

Q: What do I do if I have a scab that won't go away one week after Fraxel Restore that was done in NYC?

A: You should contact your doctor as it is rare to have any scabs after Fraxel Restore. There are minimal scales that peel away after Fraxel Dual which helps lessen pigmentation. To ensure there is no infection, whether bacterial or viral [herpes], you should see your doctor.

Q: If I have Fraxel Dual in NYC for facial resurfacing and to even the tone of my brown spots, can I still have Ulthera for tightening?

A: Yes. Ultherapy does not preclude you from having Fraxel Dual or Vice Versa, but you should do the Ulthera before not immediately after the Dual. The reason is that the Dual causes some flaking and shedding of skin in the first week and you would rather not subject this weakening state to resurfacing, but as the Ulthera penetrates the skin and affects the dermis to lift and tighten, once it's done, the Fraxel can be done on top.

Q: My dermatologist is in NYC. What is the best cream to use after Fraxel laser?

A: It depends if you mean Fraxel Clear and Brilliant, Fraxel Refine, Fraxel Restore, Fraxel Dual or Fraxel Repair. The more mild Refine and light Fraxel Restore may only need good moisturizer and sunscreens, but the moderate Restore and Dual may need good coverage such as TNS Restorative Ointment and TNS Ceramide Creams. The aggressive Fraxel Repair laser treatments may need more occlusive treatments and even antibiotic, antiviral treatments to prevent problems depending on your skin's condition and history. Your doctor should recommend a specific regimen for you.
