Q: Should I have Juvederm, Restylane of fat injected for my tear troughs?

A: Restylane is thought be some to cause less excessive swelling in the eyelid area than Juvederm, but no studies have proven this. They are safer than fat injections as both Restylane and Juvederm can be dissolved if needed by using hyaluronidase enzyme.

Q: I thought me and my friend had the same problem of the aging face but she had Restylane injected and the same doctor suggested I have Radiesse and Juvederm in different areas. How can that be?

A: A sign of a good cosmetic doctor is if they have an aesthetic eye to see the differences, even if subtle, in different people and to know what filler is safe and good to use in a certain area. It is quite common to use different fillers throughout the face as some are too thick for some superficial areas and the superficial fillers are not thick enough for the deep valleys. Softer fillers are not as strong to lift angulated creases as more particulate fillers. Sometimes, deep fillers are used first and the superficial fillers are injected by being laid on top of the deeper filler.

Q: I had my tear troughs injected with Restylane and love the result so I recommended it to a friend of mine. She had it done by a different doctor and now has a big lump under one eye, two months later. I feel terrible for her since I recommended it be done. Is this a bruise and can it be treated?


A: It may be filler that created an area of overcorrection. It could be a small hematoma, or collection of blood, in the deeper tissues. It may resolve by itself. If it is the Restylane that was overinjected, it can be dissolved with the injection of hyaluronidase.

Q: I had Restylane injected in my smile creases but the doctor said my anatomy is such that some of the filler moved into the upper skin-colored region of the lip. He used an injection to dissolve the filler. Was this appropriate?


A: There are occasions in which filler is seen moving along a tract that is not commonly present. Immediate massage by the physician can shift this or evenly distribute the filler so that it is not seen as a lump. If the lump doesn't go away with the massage, then hyaluronidase enzyme can be injected to dissolve the Restylane or Juvederm (another hyaluronic acid which is similar to Restylane).
