Q: I got a bad bruise after Restylane™ injection for my tear trough. What can help it go away faster?

A: It usually takes about two weeks for a black eye to go away. After a couple of days of having it, not before, a warm compress can help reduce the coloration faster. Topical Vitamin K cream may help change the iron from the red blood cells to a different form of its molecular structure and allow it to dissolve faster. Arnica Montana, an herb, may hasten the lightening of the bruise coloration. Some people try to use makeup to hide the bruise but have difficulty camouflaging it well. I am fortunate to know of a great Make-up artist who will make house calls in NYC and Long Island to my patients who have bruising or hypertrophic scars after reconstruction from Mohs surgery and the patients who have developed a bad bruise from Perlane, Juvederm UltraPlus, Radiesse, and Fat Injections. These are thick fillers and are placed deeply in the dermis and is slightly more prone to significant bruising. It is rare to have a bad bruise from Restylane™, Juvederm Ultra and Botox / Dysport injections.

Q: I had a filler injected in my lips and notice a bump now that wasn't there before. I think it's the filler. What can be done about this?

A: Depending on how long ago the filler was injected, it can be massaged out, often, in the first day. If it is Juvederm or Restylane™, then hyaluronidase can be injected to dissolve the filler. Sometimes the filler is lanced and expressed by the doctor. Other fillers are more difficult to extract and are not amenable to being dissolved, but the sooner you see your doctor, the better, to try to improve the appearance.

Q: I have very bad puffiness of my lower eyelids. Should I have Restylane™?

A: It depends on your anatomy. If the swelling is related to fat herniation, then a surgical blepharoplasty would be the best treatment. Sometimes, swelling is just fluid retention, and the Restylane™ could make this worse as it helps fill by binding fluid (your water). If the swelling is seen because of a groove that is called a tear trough, and is not true swelling, but the normal skin that is higher than the groove, then if the groove is filled with Restylane™, you may improve the contour.
