Q: I am 40 years old and notice that smile creases are deepening. I don't want a face lift. What can be done?A: Often, facelifts do not address smile creases. These creases worsen over time as there is loss of the structural support of the mid cheek when the fatty tissue thins out. The skin then falls related to gravity because there is less outward projection of the contour of the cheek. This is analogous to holding a blown balloon sideways in your fingertips and then releasing the air and seeing the balloon fall down. When the cheek does this, it pushes on the corners of the lip and folds over the smile crease making it a deeper furrow. Jowls start to form over the jawline. Volumization is done in the fatty compartment to help re-support the infrastructure of the three dimensional face. Radiesse™, Fat Injections used as grafts, Perlane, Restylane™, Juvederm™ UltraPlus and Juvederm™ Ultra can be used for this purpose. Then after this is addressed, the finer areas such as the triangle below the corner of the lips can be injected by focal smaller injections of Restylane™ or Juvederm™.
Q: I have heard that I need to massage after I have Restylane™ injected. Is that right? I don't want to move it around.
A: You should not be able to move the Restylane™ or Juvederm™ filler easily after the first day, but massaging is not necessary! I usually examine the areas I inject and, by feel, ensure that it is a smooth result prior to discharging my patients. Sculptra™, which is a volumizer, is different. Many physicians recommend for their patients to massage the area that has Sculptra™ injected, but this is not an absolute necessity as there has not been an exhaustingly good study to convince me that massaging is helpful in reducing Sculptra™ bumps. I prefer my patients not to massage the areas I fill with Restylane™, Juvederm™, or Radiesse™.
