Q: What is the best laser to treat my sundamage and wrinkles?A: Fraxel Dual V-beam, Alexandrite, Neodynium Yag lasers are the best for reduction of brown facial spots. A dermatologist should be consulted to ensure the lesions are OK to treat.
Q: I hate my lower eyelid creases. What is usually done for these?
A: Laser resurfacing provides an efficient treatment for these wrinkles usually. No treatment can get rid of 100% of the lines as the squinting we do makes the skin crease over the underlying folding of the contracting muscle. One cannot undergo much Botox treatment on the lower eyelids without compromising eyelid function. Skin surgery, chemical peel and laser resurfacing can be done. Portrait Plasma resurfacing helps this skin with about one week of down-time but provides a natural-appearing skin surface, wrinkle improvement and some tightening.
Q: If I am not wrinkled but do have sundamage with freckling and uneven skin tone, can I have Fraxel?
A: Fraxel Dual is a great treatment to help reverse sundamage. Certainly the use of sunscreen is very important. Topical therapy with antioxidants may help, the use of Retin-A cream can improve the skin coloration too. Fraxel Dual creates some light shedding of the dark skin and after a week the skin develops a new healthy glow. One is not home-bound during the recovery. Special creams are used to help you heal well. One should avoid the use of Retin-A for some time prior to the laser and during the Fraxel Dual and for two weeks after approximately.
Q: I want to have the best reduction of my wrinkles and I have the time to take off to heal. What is best for me to do, some laser or chemical peel or microdermabrasion?
A: No doubt that resurfacing with a minimal of medium depth chemical peels, Portrait Plasma in our NYC practice, Fraxel Repair laser and dermabrasion, not microdermabrasion, can effect a wonderful transformation in many sundamaged and aged patients. Deep chemical peels are hardly done any more and have a greater risk than medium peels but the latter do not produce as good a wrinkle reduction as Portrait Plasma or Fraxel Repair laser. Each patient has different skin, different risk profile, different medications that they may take, etc. A consultation is important for the physician to see the skin tone and texture and coloration. Then after taking a thorough history, the physician can evaluate and recommend the best treatment, while discussing the risks, advantages, alternatives and convalescence including down-time and follow-up care.
