Q:I have ulcerative colitis but I want my smile folds to be injected with Restylane. Is that a problem?

A: No. Fillers, such as Restylane, Juvederm and Radiesse, are injected in patients with inflammatory bowel disease such as ulcerative colitis. I have had quite a few patients with this disease and crohn's disease that have had fillers injected without any problems. You should feel secure that you can have these fillers injected. In the olden days, collagen injections were injected safely in patients with inflammatory conditions but semantics created confusion with patients that had collagen vascular disease such as lupus, but there is no problem injecting the currently used Juvederm and Restylane in these patients.

Q: I had a surgical procedure on my lower eyelid for a skin cancer and now there's a crease a half inch below the eyelashes. Can this be filled in with Restylane?

A: Lower eyelid skin creases might be too superficial to inject without seeing a lump or even a blue color from the Restylane. Sometimes we can inject more deeply, such as below the muscle, to give a generalized lift to the skin and the overlying superficial creases improve. An advantage to using Restylane or Juvederm for this is that it can be reversed by injecting hyaluronidase.

Q: I had Restylane injected in the lower skin of my eyelids four months ago, and now my eyes are irritated and burning. Is this related?

A: Probably not. You should see an ophthalmologist to examine your corneas.

Q: I had a doctor inject Restylane in my tear troughs and now have lumpiness in the skin one week later. What should I do?

A: You should return to your doctor so he or she can determine what is wrong. If there is overcorrection or too superficial placement, then the filler can be dissolved by injecting hyaluronidase. That is why I inject below the muscle, not above, otherwise it will be more readily seen and felt underneath the very thin skin in this area.

Q: I had restylane in my lower eyelids injected more than a year ago and I still think I have improvement. I thought it was supposed to dissolve before a year, could this still be the filler working?

A: Yes, many of my patients note augmentation continuing beyond a year and I am able to feel it on exam corroborating their belief. This occurs in the smile folds and tear troughs of the lower eyelids commonly.
